1. Collection of your personal information
   We will ask you when we need information that personally identifies you (personal information) or allows us to contact you. Generally, this information is requested when you are completing our “contact us” form or our patient questionnaire.

   We may use your personal information for the following purposes:
   a. If you have requested information we may send it to you via email, telephone or mail.
   b. During a consultation appointment we may use your personal information to determine if you are a candidate for Lap-Band surgery or the Gastric Balloon procedure.

   Personal information collected from obesitysurgery.ca will be securely stored at the Surgical Weight Loss Centre. We will not sell, loan, or rent any personal information that we collect to any other organization. We may disclose personal information about you to other companies or people when:
   a. We have your authorization to do soa. We have your authorization to do so
   b. We need to in order to service a request that you have made
   c. We respond to court orders or some other legal process

   Web site users may access any personal information that we have collected about them by making a request to the email address provided at the beginning of this document.    If you complete our questionnaire, the data we collect will be used exclusively for purposes related to your inquiry. The Surgical Weight Loss Centre will not give, sell, or loan any personal information collected during the contest to any other organization.

   If we make significant changes to our data handling practices a notice will be posted on our web site and, we will make efforts to notify our active customers in other reasonable ways that may include an administrative email or other notice. If the change involves the use of your personally identifiable information, the notice will contain instructions on how you can opt-out of such use.
